REQUIREMENTS:
+ 2x2 Picture
+ 2 Valid IDs (Clear copy of PRC ID or Proof of Renewal, if expired)
+ Latest 3mos Bank Statement (CA/SA) and Patient's Ledger
+ Proof of Billing (latest residential Electric or Water Bill)

Co-maker requirements waived if 10 years in practice and affiliated with various hospitals (subject to CRD approval); dentists & vets exempted

Interest Rate (Add-on): 2% per month 

Processing Fee:
+ Metro Manila: 5% + P150
+ Province: 5% + P200

Terms:3 to 12 months
