Requirements:

• Completely filled up Application Form
• 2x2 Picture
• 2 Valid IDs (Clear copy of PRC ID or Proof of Renewal, if expired)
• Copy of latest bank statement (CA/SA) for at least 3  months or list of clients
• Proof of Billing
Co-maker requirements waived if 10 years in practice and affiliated with various hospitals (subject to CRD approval); dentists & vets exempted

Interest Rate (Add-on):  2% (for doctors) or 2.5% (for dentists and vets)

Processing Fee:

• Metro Manila: 5% + P150
• Province: 5% + P200

Terms:  3 to 12 months
