Requirements:
+ Completely filled up Application Form
+ 2x2 Picture
+ 2 Valid IDs (Clear copy of PRC ID or Proof of Renewal, if expired)
+ Copy of latest bank statement (CA/SA) for at least 3 months or list of clients
+ Proof of Billing

Interest Rate (Add-on): 2.5% (for dentists and vets)

Processing Fee:
+ Metro Manila: 5% + P150
+ Province: 5% + P200

Terms:3 to 12 months
