I. LOAN APPLICATION FORM

Duly accomplished and signed Application Form
II. INCOME DOCUMENTS
IF LOCALLY EMPLOYED
Certificate of Employment indicating salary, position and length of service
Latest Income Tax Return
IF OVERSEAS FILIPINO WORKER/SEAMAN
Certificate of Employment duly authenticated by the Philippine Consulate
Crew contract and Exit Pass validated by POEA (for seaman)
Proof of monthly remittances for the past six (6) months
Notarized Special Power of Attorney or Marital Consent (if spouse is out of the country)
IF SELF-EMPLOYED
Articles of incorporation/By-laws/SEC Registration
Audited Financial Statement for the last three (3) years
DTI Registration
Bank statement for the past six (6) months
Latest Income Tax Return
List of trade references (at least 3 names with telephone nos.)
IF FROM COMMISSION
Vouchers or Bank Statement (for the last six (6) months reflecting commission income)
IF FROM RENTAL PROPERTIES
Rental/Lease Contract
Photocopy of proof of ownership (TCT, OR/CR, etc,)
III. COLLATERAL DOCUMENTS
Photocopy of OR/CR
POST APPROVAL REQUIREMENTS
Original OR/CR under the name of the borrower
Original comprehensive insurance policy with official receipt or certification of full force
Post dated checks covering the whole term of loan
Photocopy of PNP Clearance, Deed of Sale and Macro etching (if purchased)
3 complete and readable stencils in bond paper
3 complete and readable stencils in LTO blue form
Signed Promissory Note with Chattel Agreement, Disclosure
Statement, Authority to Release Proceed and Undertaking
Valid identification cards
Tax Identification Number (TIN)
Photocopy of Community Tax Certificate or sedula
List of personal references (at least 3 names with telephone nos.)
