+ Stencil of motor number and chassis number (in blue form)
+ LTFRB Franchise
+ TIN ID or any proof of TIN & Cedula
+ Proof of Income
------ DTI, Business Permit & Latest 3mos Bank Statement (if Single Proprietor)
------ SEC Reg., Articles of Inc., Business Permit, Board Resolution allowing the corporate representative (major stockholder) to apply for a loan & Latest 3 mos. Bank Statement
+ Latest 2x2 picture (3pcs)
+ Photocopy of 2 valid IDs (Company ID, Passport, Driver s License, SSS, etc.)
+ Proof of Billing (electricity, telephone, water bills)

Loanable Amount : Max. of 200K (up to 20% of the appraised value)
Processing Fee : 5.5% of amount financed
